    • Job Requirements
    • MUST have director experience at hospital > 150 bed, managing large teams (approx 20)
    • Acute, LTAC, SNFs, and TCU Experience (CM Director will need to keep Transitional Care Unit full)
    • MUST have EPIC experience, InterQual, discharge planning, Conditions of Participation, CMS guidelines required
    • Will need to provide education and guidance with providers
      • Schedule Information
      • Weekend Hours: Every other weekend
      • Call Requirements: No formal call. Available for department needs
        • Unit Specific Information
        • Facility Department/Cost Center: Case Management

        • Reason for Interim: Need to make immediate CM impact within 30 days of start

        • Top objectives include Monthly executive review with hospital and division leadership, Implement necessary changes for immediate impact on metrics
